Disciplines supported include Physical Therapy, Physical Therapy Assistant, Occupational Therapy, Certified Occupational Therapy Assistant and Speech Language Pathology. The role will use a combined skill set of clinical knowledge, customer service, problem solving as well as coordination among crossfunctions. Although this is an office based position, this role will have a direct impact on the quality of services provided by the Rehabilitation Team.Provides education and reinforcement regarding adherence with internal processes and procedures as well as federal and state guidelinesReceives case referrals, reviews available patient information related to the case, including disciplines required to determine home health needsReviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is generally available at all times to assist clinicians as appropriateAssists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals. Assists in the orientation of new organization personnelKeeps track of recurring adverse situations and works with the DOR to create additional educational opportunities and resourcesPrepare and present educational materialsCommunicates and establishes positive relationships cross functionally in an effective and efficient manner with a focus on problem resolution.
